|Safety Incident Reporting - Use of Information|
Flow of Information
Once completed, the on-line incident report will be forwarded automatically to the IMA Executive Vice President who will make it available for review by a select group of the safety committee. The purpose of the review is to determine if further follow-up is required before the incident report is released for distribution.
As a reminder all identifying information is confidential and names of companies or organizations, locations, and the like will be removed to ensure confidentiality
Upon completion of the review the incidents will be made available on the IMA website in an area called “Safety Incidents” that can be accessed by registration of name and email address.
Each year the IMA Safety Committee will publish a report based on the incident reports gathered and reviewed. The objective will be to report the types and severity of incidents in a summarized form. Additionally, any industry trends will be addressed.
The yearly report will be published in the monthly IMA newsletter and made available on the IMA website. Further distributions will be at the discretion of the IMA executive committee and Board of Directors.
Finally, while every effort is made to verify the accuracy of information contained in the incident reports, no guarantee is expressed or implied with respect to the completeness, casual attribution, or suggested remedial measures for avoiding future events of similar nature.
